NOTIFICATION AFTER THE TEST

Send this form once more again to caa@caa.cz together with the following enclosures:

  • copy of the Skill Test, Proficiency Check or Assessment of Competence Form (Forms from other EASA member states are accepted)
  • copy of the applicant´s pilot licence (only after a hand-written entry)
  • copy of the applicant´s medical certificate (only when a hand-written entry into the pilot licence has been made)
  • copy of the simulator certificate (if applicable)
  • copy of ATO/FTO/TRTO recommendation for the proficiency check (if a hand-written entry of renewed type/class/instrument/instructor rating was made into the pilot licence)
  • a report of the examiner detailing the reasons for any failure, partial pass, or any special circumstance (if applicable)
